摘要(Abstract)：

慢性心力衰竭(CHF)作为现代医学中最难攻克的难题之一,是心血管疾病的重要致死原因,在全球医疗支出中占据着极大的比例,笔者对既往成功实行的有中医治疗特色的CHF临床路径进行回顾性分析,古代中医文献资料整理、现代CHF文献资料整理等基础上,构建一个促进患者自我管理、综合医院和社区资源互动的,以预防为主的,终身随访、全程跟踪的心衰慢性病管理体系。
