灯盏花素对慢性阻塞性肺疾病急性加重期免疫功能影响的Meta分析A Meta-analysis of RCTs:Effect of Breviscapine on Immunologic Function in AECOPD

摘要(Abstract)：
目的系统评价灯盏花素对慢性阻塞性肺疾病急性加重期免疫功能的影响。方法通过计算机检索国内外数据库(如Pub Med、Embase、Web of Science、The Cochrane Library、CNKI、万方、VIP、CBM),选取灯盏花素对慢性阻塞性肺疾病急性加重期免疫功能影响的临床随机对照试验。由2位研究人员独立检索,筛选文献,并根据Cochrane系统评价方法学对纳入的文献进行资料提取和质量评价,采用RevMan5.3软件进行Meta分析。结果最终纳入7篇文献,共624例受试者。Meta分析结果显示与对照组相比,治疗组疗效确切,体现在免疫细胞水平含量提高,包括CD3+T细胞[MD=5.47,95%CI(4.69,6.25),P <0.00001]、CD4+T细胞分别为[MD=10.41,95%CI(9.02,11.08),P <0.00001]及[MD=3.41,95%CI(2.64,4.18),P <0.00001]、CD4+T/CD8+T[MD=0.62,95%CI(0.95,0.65),P <0.00001]、IgA[MD=0.49,95%CI(0.37,0.61),P <0.00001]、IgG[MD=0.96,95%CI(0.48,1.43),P <0.0001];并下调CD8+T细胞[MD=-3.02,95%CI(-3.73,-2.66),P <0.00001],而对IgM,两组无明显差异。另一方面,临床有效率[OR=3.02,95%CI(1.53,5.95),P=0.001]提示优于对照组。结论灯盏花素改善慢性阻塞性肺疾病急性加重期患者免疫功能方面有一定的疗效。
