PICCO监测下参附注射液对脓毒症休克早期血流动力学的影响Effect of Shenfu Injection on Hemodynamics in Early Stage of Septic Shock by PICCO Monitor

摘要(Abstract):
目的观察参附注射液对脓毒症休克患者早期容量复苏的血流动力学影响。方法将60例脓毒症休克患者按随机数表法分为参附治疗组和标准治疗组各30例,标准治疗组按照指南给予标准治疗,参附治疗组给予标准治疗加用参附注射液治疗。在早期容量复苏治疗过程中,应用PICCO监测仪观察患者6 h和24 h的血流动力学指标(CI,SVV,SVRI,GEDI,ELWI,MAP),并监测患者第0、6、24小时的血气指标(Lac,BE,DO_2,VO_2,ScvO_2)。结果治疗后参附治疗组患者6 h的CI、SVRI、MAP、BE、ScvO_2均较标准治疗组明显升高(P<0.01),SVV、Lac、DO_2、VO_2较标准治疗组明显下降(P<0.01),治疗后24 h的CI、SVI、SVRI、MAP、BE、ScvO_2均较标准治疗组升高(P<0.05),SVV、Lac、DO_2、VO_2较标准治疗组下降(P<0.05),但两组患者治疗后GEDI、ELWI比较差异无统计学意义(P>0.05)。结论参附注射液在脓毒症休克早期容量复苏治疗中对血流动力学有明显的改善作用。
