慢性心力衰竭发生心肾综合征的临床特点及相关危险因素分析Clinical Characteristics and Risk Factors of Cardiorenal Syndrome in Patients with Chronic Heart Failure

摘要(Abstract):
目的分析慢性心力衰竭(CHF)住院患者发生心肾综合征(CRS)的临床特点及相关危险因素。方法选取147例CHF患者,根据eGFR值是否<60 mL/(min·1.73 m2)分为CHF组65例和CRS组82例,收集患者一般临床资料及化验检查指标,采用Logistic回归对CHF伴发CRS的危险因素进行单因素和多因素分析。结果两组患者在年龄、性别、心功能分级、高血压病史方面差异均有统计学意义(P <0.05或P <0.01)。CRS组患者血Hb、LVEF水平明显降低,PLT、UA、血脂、左室壁厚度明显增加,与CHF组比较差异具有显著性统计学意义(P <0.05或P <0.01)。Logistic回归显示,高龄、女性、高血压病史、入院时PLT、LDL-C、UA升高,LVEF值降低是CHF发生CRS的高危因素。结论高龄、女性、高血压病史、入院时PLT、UA、LDL-C水平升高、LVEF值降低的CHF患者容易发生CRS。
